Instructions
Title Sponsors are available for each month’s Third Friday event. Please contact Main Street Warsaw and/or the Third Friday’s logistics coordinator for a listing of available months.
*Not-For-Profit Vendors are encouraged to limit sizes of free items to sample sizes. Additionally, while fundraising is not allowed, a goodwill donation for free samples is acceptable. Additionally, we encourage Not-For-Profit vendors to develop an activity that supports the theme of the monthly Third Friday.
*We have the right to limit the number of vendors with similar products/services. Third Friday registration is nonrefundable, but transferable to another event date.

Main Street Warsaw will not be held responsible for any damages, theft or injury to any person or persons attending, buying or selling at this event. The attached contract must be signed and returned with this form and payment.

Please submit your registration form by 5:00PM on Monday the week of Third Friday, payment is required 24 hours prior to the event. Third Friday registration is nonrefundable, but transferable to another event date. Thank you!
